Even if you are a local resident of Bangkok, you are encouraged to submit this form prior to your first consultation.

A telephone or video conference may be arranged in a complicated cosmetic surgery case for international patients, but is not the definitive opinion. The definitive opinion or diagnosis for any plastic surgery is given only after an examination by Dr. Chartchai in person in Bangkok, Thailand.
